What forms do I need?

***New Sports Physical Documents

The New Jersey Department of Education (NJDOE) via the NJSIAA has provided newly updated procedures and processes for Sports Physicals.

 The old physical paperwork will no longer be accepted for sports clearance.

Please read the following instructions carefully and make sure all papers are completed as instructed  to assure your child is eligible to participate in sports. Missing or incomplete paperwork will result in your child not being able to participate.

All students need to have an Updated Yearly Physical on file with the school nurse every single school year.  If a physical has already been submitted a Health History Update Questionnaire (HHUQ) form needs to be completed and turned into our school nurse. If you are unsure as to when your physical expires please check the Parent Portal on Genesis. 

If you do not have a updated physical on file in the high school, please take the following steps:

1. Please click the links below to print the following forms and take to your Primary Care Physician
   • Preparticipation Physical Evaluation History Form (this form stays with doctor)
     • Athletes with Disabilities
   • Preparticipation Physical Evaluation Physical Examination Form (this form stays with doctor)
   • Preparticipation Physical Evaluation Medical Eligibility Form (this form is turned into school nurse)
   • Health History Update Questionnaire (this form is turned into the school nurse)
2. Please make sure all forms are completed, signed, and stamped before submitting to the school nurse. You can drop off these completed forms in the black physical box in front of the school, or hand into the school nurse.  Please do NOT email any forms.
3. You will receive notification in Genesis once your child is approved to participate in sports. You will see this on both the Athletics tab in Genesis and then again in the letters portion of your parent portal.
