    Required Forms for Athletic participants and Marching Band participants

     

    All paperwork must be submitted by the deadlines listed or the student will not be able to participate in practice.

     

    What forms do I need?

    The student has not had a physical within 365 days of the first practice.

    The student has a current physical on file. Student must submit this Health History Update Questionnaire for each sports season.

    Student needs Pre-Participation History, Physical  and Clearance Forms

    If student has special needs, must also submit the Pre-Participation Physical Evaluation: Athletes with Special Needs Supplemental History Form

    All VHS Students and Parents must complete these forms on

    Genesis parent portal (forms tab) once per year.

    - NJSIAA Steroid Test Policy Consent Random Testing

    - Sports-Related Concussion and Head Injury Form

    - Student Athlete Contract (NOT required for Marching Band)  

    - Sudden Cardiac Death-Young Athlete Acknowledgement 
     (Please read Sudden Cardiac Death Pamphlet before signing)

    All Parents of VHS Student Athletes

    The VHS Athletic Department utilizes the ImPACT (Immediate Post Concussion Assessment and Cognitive Testing) computerized exam to help assist in the diagnosis and manage of concussions.  All VHS Student Athletes will obtain a baseline ImPACT test once every 2 years. The ImPACT test will be repeated if a concussion is suspected.  If you do not wish to have your student monitored for a potential concussion, please sign the Decline impact testing form (the signed form may be scanned and emailed to the coach or handed directly to the coach by the Student Athlete.)
