    Medication in School

    Students who require medication (over the counter and prescription) in school must have a written order from their health care provider. Please submit all medications and physician's orders directly to the school nurse.

    • Diabetes Medical Management Plan
    • Consent to administer medication at school (This form should be used for medication that needs to be administered during the school day (ex-ADHD medication, pain medication, antibiotics, prescription topical ointments....) It is highly recommended that the Life Threatening Allergy Action Plan be used for students who require Epipens and the Asthma Treatment Plan be used for students with asthma.)
     

     

     

    All medication must be picked up at the end of the school year.
